Where on the face does acne usually show up?

In a 2011 multicenter study of 1,236 acne patients in Korea, the most common areas were the forehead and cheeks, and acne on the cheeks was more severe. The Korean Dermatological Association explains that acne that continues past age 25 or starts after it tends to appear on the chin, jawline, and neck.

Acne often starts on the face, where oil glands are packed together, and can spread to the neck, back, and chest. But the sources gathered for this guide include nothing that lets you read “it’s on my forehead, so it must be this.” Knowing the common areas is only useful for deciding how to split your log into columns.

If spots keep coming back in the same place, what should you write down first?

Start by writing, right next to the location, whatever stayed on that spot for a long time that day. Even for the same right cheek, a day with an hour-long phone call and a night spent sleeping on your right side involve different things touching your skin.

Here is how that looks in practice. An office worker who takes calls on the right ear writes call time in the right-cheek column. A student who rests their chin on one hand during online lectures marks the jawline on that side, and someone with bangs marks the forehead column on days they washed their hair. Anyone who shaves circles shaving days in the chin column.

Do not skip areas that look clear. In the 2018 follicle study, even forehead skin with no lesions had more follicles with a thickened border than controls (68%), and in a different group of 15 women with jawline acne, the lower jaw had more such follicles than the same women’s forehead (76%). The day a spot first appears in a column that used to be clear often turns out to be the most useful entry.

Can touching your face cause acne in certain areas?

Hand habits are the item that seems to line up with location most easily, but the sources for this guide include no study that directly measured how often people touch their face against how many pimples they get. So it is more accurate to check with your own log than to assume.

What is clearly stated is the effect of scratching or squeezing. The Korea Disease Control and Prevention Agency (KDCA), Korea’s national public health agency, explains that scratching or squeezing acne yourself makes scarring more likely. Keep separate columns for “touched” and “squeezed,” and mark the day you rubbed your chin in a meeting apart from the day you pressed on your nose in front of the mirror.

Where does stress acne appear?

We did not find evidence that stress decides a particular spot. In the 2011 Korean study, stress, lack of sleep, and periods were named as triggers that made acne worse, but they were not surveyed against face areas.

During exam season or deadline weeks, sleep tends to shrink while time spent resting your chin or touching your face tends to grow. If you log only locations, that overlap can create the illusion of a “stress spot.” A separate conditions column lets you look at the busy week itself apart from the hand habits that increased that week.

After 2 weeks, add up the counts for each zone and pick only the one with the most. For the next 2 weeks, change the one item written most often next to that zone: switch phone calls to earbuds, or move your resting hand to the other side. If you change two things at once, you are back to not knowing what made the difference.

When should you bring your location log to a dermatologist?

  • If three or four back-to-back 2-week logs add up to 6 to 8 weeks and the count has not gone down, bring your log to a dermatology appointment.
  • If deep lumps that hurt when pressed form on your cheeks or chin, consider getting seen. KDCA explains that lesions in these areas tend to be deep and carry a higher risk of red marks and scars.
  • If your nose and cheeks stay red and burning, or a fine rash spreads around your mouth, it may be a condition that needs to be told apart from acne, such as rosacea or perioral dermatitis, so see a dermatologist to check it.
  • If spots increase on your face, chest, and back all at once regardless of location, keep logging and also talk to a medical provider.
