# Skin Problems During Pregnancy: Adjust Your Routine by Trimester

> The skin changes that are common during pregnancy shift from one stage to the next, so instead of adding products, cut your routine back and bring a list of ingredients to your doctor.

Skin problems during pregnancy shift from early to late weeks and after birth. When acne, melasma and stretch marks change, what to cut back, and what to ask.

| Early pregnancy (first 3 months) | Many people notice new acne or more of it | NHS 111 Wales · official |

## Early pregnancy: if breakouts increase, what do you cut first?

Use fewer products, and check with your OB-GYN before using anything labeled with a retinoid, such as retinol or retinal. This guide does not rule on what is safe or risky; your job at this stage is to collect photos of ingredient lists and show them at your appointment.

*Early on, jot down one line for each area that flared and the week of pregnancy.*

## Melasma in the second trimester: can aloe gel carry you through?

The Korean Dermatological Association says melasma mostly appears during pregnancy or after starting birth control pills and darkens in summer, so sunscreen as the last morning step and a log of time in the sun come first. There is an aloe study that split 180 pregnant women into groups, but it used a special liposomal formula, so the results do not carry over to the aloe gel you have at home.

*Note the month and area where your skin got darker, plus how long you were in the sun that day.*

## After birth and while breastfeeding, what do you add back?

Even while breastfeeding, bring back new ingredients one at a time, after you have talked with your doctor. For leftover melasma or stretch marks, taking a photo once a month in the same light makes it easier to describe the change at your visit.

## Routine changes by stage

| Stage | Cut back or pause | Keep | Bring to your appointment |
|---|---|---|---|
| Right after you learn you are pregnant | Products labeled with retinoids, adding several new products at once | Gentle cleansing and moisturizing | Photos of the ingredient lists of what you use, list of medicines you take by mouth |
| Second trimester | Long stretches in the sun | Sunscreen | Month and area where melasma got darker |
| Third trimester | Layering several strongly scented body products | Moisturizing the belly and breasts | Week of pregnancy when stretch marks appeared |
| After birth and while breastfeeding | Restarting several ingredients at once | Adding things back one at a time | Monthly photos, whether you are breastfeeding |

*In the third trimester, as the belly grows, keep moisturizing the belly and breasts and simply note the week stretch marks showed up.*

## When should a doctor weigh in?

- If painful lumps increase or start leaving marks, see a dermatologist and tell them how many weeks pregnant you are.
- If you learn you are pregnant while taking an oral acne medicine, contact the clinic that prescribed it right away.
- If melasma or stretch marks are unchanged several months after giving birth, ask a dermatologist about your options.

## Evidence at a glance

- In a 1991 double-blind trial that split 80 pregnant women into groups (PMID 19291041), 34% of the group using a combination cream with Centella asiatica extract developed stretch marks, compared with 56% of the placebo group.
- In a 2017 trial of 180 women with melasma during pregnancy (PMID 28139161), melasma scores after 5 weeks improved by 32% in the liposomal aloe group and by 10% in the regular aloe group.
- In a 2022 trial of 64 women with pregnancy stretch marks (PMID 35570397), the group that paired beta-glucan with laser improved the most, while beta-glucan alone brought only slight improvement.

## Frequently asked questions

### What should I do if I break out during pregnancy or after giving birth?

Do not bring in several new products at once; write down the week of pregnancy, the area, and the products you were using when breakouts increased. NHS 111 Wales, part of the UK's public health service, says acne in pregnancy usually shows up in the first three months. Taking these notes and photos of ingredient lists to your OB-GYN or dermatologist makes it easier to decide what to stop.

### Can I use PDRN skincare while pregnant or breastfeeding?

The sources checked for this guide include no study of pregnant or breastfeeding women using PDRN cosmetics. There is no data to call it either fine or risky, so if it is a product you already use, show the ingredient list to your OB-GYN and ask first.

### Can I use acne medication while pregnant, breastfeeding or as a teen?

Whether to use acne medicine is always a question for the doctor who prescribes it. A 2012 observational study that followed 79 women exposed to an oral acne medicine (isotretinoin) around pregnancy concluded that caution is needed, since risks were reported even with low-dose exposure. If you learn you are pregnant while taking it, tell the prescriber right away.
